Today, SMG continues to grow with the addition of nurse practitioners to the team. Dr. Siddiqui and her team follow patients through every step of the healthcare journey. Signature Medical Group is affiliated with several hospitals and senior facilities in the north and northwestern suburbs of Illinois.
When admitted to the hospital, or rehabilitation center, we continue to monitor and care for you. Should you decide to move to a nursing facility, we also follow you there.
Our goal is to provide comprehensive medical services conveniently for you, from laboratory testing and flu shots, to physical therapy (coming soon).

I am a board-certified family practitioner. In 2007, I completed my Family Medicine residency at Mount Sinai Hospital. I am passionate about providing individualized treatment plans and preventive care for all patients: the old-fashioned way, with a modern touch.

Dr. Siddiqui is the PCP for my mom. She is a compassionate, caring and a very knowledgeable doctor. I am very thankful for her compassionate and articulate manner in which she addressed the issues that my mom brings forth. I just read some of the reviews and I’m simply appalled that most of the negative reviews seem to actually contradict themselves. They speak of things that clearly Dr. Siddiqui was doing were for the best interest of the patient. In my experience of dealing with Dr. Siddiqui and her staff, I’m highly impressed with their timely and prompt responses. I can't tell you enough how Dr. Siddiqui has always been their for me and my husband. At a drop of a dime I have called crying and they have dropped everything to help me and my situation. She is very caring and will help you tackle what needs to be done. In my life time I have seen a few doctors. She is by fare the best at what she does. Her staff is always on the ball. Dr Siddiqui is very knowledgable not only in her own field, but almost all fields. I thank Dr Siddiqui and her whole group for being so professional, courteous, and polite. They answer all my questions, collect all papers from different doctors and labs. They try their best to get me feel better. Absolutely love her!!! We have known her for a decade and all my family goes to her. I trust her with my close eyes. My husband was suffering with a rare condition and all his life he has been trying to find the dx and dr. Siddiqui was the one that did actually helped us. She has helped me with my father and now I am under her TX. Her office is professional and take cares of stuff in a timely manner. Highly recommend her if u are looking to find a cure.
